Hospice of the Valley is pleased to offer you the opportunity to host a pop up event at your location to learn about a grant funded program called Choose Your Person.  This event offers Arizona residents, including winter visitors, the opportunity to register their preferences as to who would medically speak for them when they are no longer able to speak for themselves. This is very important for everyone over the age of eighteen. There is no cost or obligation.
